Part 1: Proof of Residency

Identify your living situation below and gather the required documents.

🏠 For Homeowners

1. Primary Document: Current property tax bill, mortgage statement, or closing disclosure (if recently bought).

2. Secondary Document: One (1) document with your name and address (e.g., utility bill, financial statement, voter registration, license, or court order).

🏢 For Renters

1. Primary Document: Current lease or letter from the leasing agent.

2. Secondary Document: One (1) document with your name and address (e.g., utility bill, financial statement, voter registration, license, or court order).

👨‍👩‍👧‍👦 If You Live with Another Family

1. Affidavit of Residency: Must be notarized and completed by the person you reside with.

2. Resident's Proof of Ownership/Lease: The host (person listed) must provide their current lease, tax bill, or mortgage statement.

3. Parent's Secondary Document: One (1) document with the parent/guardian’s name and address (e.g., utility bill, financial statement, voter registration, license, or court order).

Note: If your living arrangement is not listed above, please call the office for guidance.

Part 2: Student Documents

These are required for enrollment. Failure to present them may delay your child's start date.

• 📄 Proof of Age: Original birth certificate, passport, or official legal document.

• 🩺 Health & Immunization:

  • Must be current and translated into English.

  • Physical Exams: Must be dated within the last 365 days.

• 🏫 Previous School Records: Transfer card, most recent report card, and test scores (WIDA, PARCC, etc.). If applicable: IEP or 504 plan.

• ⚖️ Proof of Custody (If Applicable): Legal divorce decree or court document indicating physical/residential custody.
