Before You Begin: Required Documents


Children must be 5 years old by October 1, 2026. Please select School Year 2026-2027 and Grade KF when registering.
Part 1: Proof of Residency
Identify your living situation below and gather the required documents.
π For Homeowners
Primary Document: Current property tax bill, mortgage statement, or closing disclosure (if recently bought).
Secondary Document: One (1) document with your name and address (e.g., utility bill, financial statement, voter registration, license, or court order).
π’ For Renters
Primary Document: Current lease or letter from the leasing agent.
Secondary Document: One (1) document with your name and address (e.g., utility bill, financial statement, voter registration, license, or court order).
π¨βπ©βπ§βπ¦ If You Live with Another Family
Affidavit of Residency: Must be notarized and completed by the person you reside with.
Resident's Proof of Ownership/Lease: The host (person listed) must provide their current lease, tax bill, or mortgage statement.
Parent's Secondary Document: One (1) document with the parent/guardianβs name and address (e.g., utility bill, financial statement, voter registration, license, or court order).
Note: If your living arrangement is not listed above, please call the office for guidance.
Part 2: Student Documents
These are required for enrollment. Failure to present them may delay your child's start date.
π Proof of Age: Original birth certificate, passport, or official legal document.
π©Ί Health & Immunization:
Must be current and translated into English.
Physical Exams: Must be dated within the last 365 days.
π« Previous School Records: Transfer card, most recent report card, and test scores (WIDA, PARCC, etc.). If applicable: IEP or 504 plan.
βοΈ Proof of Custody (If Applicable): Legal divorce decree or court document indicating physical/residential custody.
