Sports Physicals will be offered at MHK on Tuesday, March 3.
In order to reserve your spot, you must submit paperwork to our Athletic Trainer by Friday, February 20. For organizational purposes, absolutely no exceptions or extensions will be made.
Physical Date: Tuesday, March 3, 2026, during the school day
Last date to submit paperwork: Friday, February 20
Submit Paperwork to: Athletic Department via email: mhkathletics@ewrsd.k12.nj.us OR by having your child put the paperwork in the mailbox outside room 509B
What to submit: FIRST 2 pages of the physical form (attached) fully completed AND signed.
Physical forms are attached to this correspondence and available in each grade level office.
