Anyone who wishes to participate in any of the athletic programs offered at HHS and MHK must register. Registration is due on Genesis by July 1st. 
We will not be offering extensions.

• All athletic registrations will be completed on the Genesis Parent Portal:  

• Sign in to the parent portal using your parent/guardian login.

• Click on “Forms”.

• Choose “HHS Sports Registration” or “MHK Sports Registration”.

• Choose a sport.

• Complete all release forms.

• Complete the Health History Update if applicable.

• You do not need to print any forms.  All information will be saved automatically.  If you return to the “Forms” page you will see a check mark and the date the form was completed.

• Please complete all questions, if you miss a question the form will not submit. 

Students registering for athletics must have a current physical examination on file in the nurse's office in order to participate. The physical must be valid on the first day of tryouts, if it expires before the first day of practice an updated physical will need to be completed by a Doctor and submitted before the deadline. The pre-participation physical evaluation is good for one calendar year (365 days) and needs to be completed by a doctor on the paper form which can be found on the HHS and MHK registration webpage and linked below:

You must register on the Parent Portal and select the sport you want your student to participate in each season by the due date unless your child is having the school doctor complete the physical portion which will be listed by season. 

• Sports Physical NEW 2024 English

• Sports Physical NEW 2024 Spanish

• All sports physicals examination dates must be within 365 days of the first day of practice.  If you had a sports physical the previous year on or after the date of the start of your season, then your physical is valid for that season – provided that it is being submitted on the below forms.  

• Make sure the Vision Exam is completed on your sports physical form prior to turning it in. If your vision exam is less than 20/40, you will be required to be corrected with contacts or glasses to better than 20/40 for athletic clearance. 

• Physical exams must be reviewed and approved by the East Windsor Regional School District Physician prior to an athlete being medically cleared for participation.  This process could take up to 3-5 days.

• Each season requires the Health History Questionnaire – Part A to be filled out by a parent/guardian before your child is cleared for participation.  Part A does NOT need to be signed by a physician.
